Who can request information regarding a sacramental record?
Under Texas privacy legislation and our Amarillo Diocesan Privacy policy, information about a sacramental record will only be released:
(1) to the person whose name appears in the record;
(2) OR to the parent or legal guardian of an individual under 18;
(3) OR if there has been prior authorization for the release of this information .
Please allow two weeks to process your request once you submit this form.
A non-refundable processing fee of $10.00 per certificate and a copy of a state-issued photo ID along with the signature of the person must accompany this form.
Personal Checks, Cashier's checks, or money orders are acceptable forms of payment. Make checks or money orders payable to: Saint Joseph Parish
